Will my insurance cover chiropractic and massage?
Many insurance companies cover chiropractic and massage therapy, but every policy is different. We’re happy to check your benefits so you know exactly what your plan covers.
What is my deductible?
Your deductible is the amount you may be required to pay before your insurance begins reimbursing for visits. Deductible amounts vary by plan. We verify this information before your visit so you’ll know what to expect out of pocket.
What are co-pays and co-insurance?
Co-pay: A fixed amount you pay at the time of service, usually ranging from $5–$50.
Co-insurance: A percentage of the billed services that you’re responsible for, often between 10%–30%.
What insurance companies are you contracted with?
Dr. Fox is contracted with: Regence BCBS, HealthNet, Providence, Aetna, and Kaiser.
Dr. Crokin is not currently contracted with any insurance companies.
We do bill all insurance companies, and many offer out-of-network coverage.
Do you accept auto or work-related injuries?
Yes. Both Dr. Fox and Dr. Crokin accept patients with motor vehicle accident or work-related injuries.
